Enhanced DBS Checks

Step- by-step guide

Physics Partners require Enhanced DBS Checks for staff, trustees and trainers. Please use this guide to help you complete your application. If you have any questions then please email sally.smith@physicspartners.com.

We use a processing organisation called DBS Check Online to carry out the checks. Your proof of identity documentation will be reviewed by Physics Partners and DBS Check Online. The documents are solely required for the purpose of submitting the check. Once submitted, these documents will be erased from Physics Partners and DBS Check Online’s systems, as per the government guidelines.

Step 1:

  1. Open the application form in a new tab by clicking here>>

a) Enter Employer details as follows:

  • Primary Contact – Sally Smith
  • Employer ID – 768584
  • Employer Email address – sally.smith@physicspartners.com

b) Enter your details under Employee details.

Step 2:

a) Answer employee questions. You must provide at least one of the requested numbers

N.B. Your driving license valid from date is section 10 on the back of your photocard or section 6 of your paper license.

Step 3:

a) Answer the questions on the ‘DBS Check Type’ page as follows:

  • Workforce type: Child and adult workforce
  • Working with children in a regulated activity? Yes
  • Working with adults in a regulated activity? Yes
  • Position applied for: Trainer / Trustee (as relevant)
  • Please select NO for the remaining questions on this page.

Step 4:

a) Review your details and check they are correct. Please note that the email address will say sally.smith@physicspartners.com – this is correct as Sally is the primary contact for your application.

b) Tick the consent boxes

Step 5:

a) Download PDF form / Download Word form

b) Print and complete the Identity Checking Form with an ID checker.

An ID Checker must be someone who: 

  • Has personally known you for at least 12 months
  • Is not related to you by birth or marriage, or in a personal relationship with you (this includes partners, in-laws, and step-parents)
  • Does not live at the same address as you

TWO documents are required for your application. The combination of documents you provide MUST show your name, address and date of birth. You must provide at least one document from group A and at least one from group B.

Group A:

  • Passport
  • Birth Certificate
  • Driving Licence
  • Immigration Document issued by the Home Office

Group B:

  • Mortgage statement – (UK or EEA – Issued in last 12 months)
  • Bank or building society statement – (UK and Channel Islands or EEA – Issued in last 3 months)
  • Bank or building society account opening confirmation letter – (UK – Issued in last 3 months)
  • Credit card statement – (Issued in last 3 months)
  • Financial statement, e.g. pension or endowment – (UK – Issued in last 12 months)
  • P45 or P60 statement – (UK and Channel Islands – Issued in last 12 months)
  • Council Tax statement – (UK and Channel Islands – Issued in last 12 months)
  • Utility bill – (UK gas or electric bill – Issued in last 3 months)
  • Benefit statement, e.g. Child Benefit, Pension – (UK – Issued in last 3 months)

c) Scan/photograph the completed Identify Checking Form

Step 6:

a) Scan/photograph your 2 pieces of ID documentation

b) Please ignore step 4 of the ‘What to Do Next’ section on the Identity Checking form. Instead please send the copies/photos of your ID documents AND the copy/photo of the Identify Checking Form to sally.smith@physicspartners.com.

Congratulations!

You have now completed the process and should receive your Enhanced DBS Certificate in the post in the next few weeks.


Update Service

We recommend that you register for the DBS Update Service. You can do so within 30 days of the certificate being issued.
